The formation of FACODE in 2006 was born out of a deep conviction by a group of highly motivated health and community development professionals to respond to the plight of children, women and men who had been impacted greatly by the 1996 Allied Democratic Forces (ADF) war and HIV/AIDS in Uganda, particularly the Rwenzori region. The idea came into play to work with communities under a premise of enhancing sustainable livelihoods among the local underprivileged community groups especially women, men and children in their social, health and economic well being.

Since formation, FACODE works to empower communities to challenge social and health evils, access development services as well as take responsibility for their development.

Based in Western Uganda in Kasese District, FACODE current geographical reach is one district of Kasese.
